Free Dilantin levels declining

I was on Dilantin for 25 years with no problems. A new doctor switched me to Keppra due to osteopenia concerns. Over 4-5 years Keppra was less and less effective even with increasing dosage. 2 years ago I went back on Dilantin but my free Dilantin levels have steadily declined, even as I have increased dose from 400 to 450 mg. Has anyone had similar issues?

I've been on Dilantin quite steadily for almost forty years now. No I have not experienced such a problem.  One question: Are you currently taking Dilantin, or has the pharmacy given you the generic form called phenytoin?  The Epilepsy Foundation has encouraged patients and their doctors to stick with the brand name version of seizure control drugs as the "generic equivalents" can vary a bit.

Thanks for your comment. I have been on the generic version for years and have not had a problem, but I will mention this to my doctor.

In generics the only thing

In generics the only thing different is the fillers. All working properties must be the same as name brand.
