Post Concussive Syndrome and Episodes

Fri, 10/12/2018 - 15:55
This is my first post here, so please forgive any possible mistakes or lack of site specific etiquette. I was involved in a car accident a few months ago. I was diagnosed with a concussion, then later on diagnosed with Post Concussive Syndrome. This has been my 7-8th concussion. About a week after the accident I started having myoclonic jerks day in and day out. These escalated to having a strong visual aura and sudden and strong muscle contractions before falling asleep. I woke up one night and I had chipped a tooth because my jaw was chattering so strong and fast. During the day, I started feeling like a wave of heat and pressure would roll through my skull followed by a complete lack of thought, and my face would droop on the left side. These daytime episodes would last about 20 seconds, while the night time episodes would last about 10 seconds but I would have 15-20 of them in clusters. After a few weeks of this, I was advised by my PCP to see a neurologist, as these might be post traumatic seizures. My neurologist said that these symptoms were atypical but decided to put me on Keppra just to see if the symptoms go away. The symptoms did in fact subside, at a cost of some pretty dark thoughts and mood swings on Keppra. After about a month, I told the neurologist that I couldn't handle the Keppra anymore but it did help with my "episodes" and myoclonic jerks. He decided to have me try Lamotrigine, which I needed to titrate up on. During the first couple weeks of titrating up, I had a few of these episodes while taking naps, falling asleep, during the night (would wake up and my hands and arms were clenched extremely tight). This all subsided once I got to 200MG a day. I ended up seeing a neuropsychologist per my neurologist's recommendation. The neurologist warned me that the neuropsychologist would most likely say it was anxiety. To my surprise, after hours of testing, the neuropsychologist told me that there is something going on in my right frontal lobe that is causing an immense decrease in attention, working memory, problem solving, and processing information. He said most likely I am experiencing seizures in the right frontal lobe and possibly a little in the right temporal lobe. After feeling episode free for a couple weeks, two mornings in a row I woke up feeling like I had a full upper body workout and didn't sleep. I ended up installing a camera above my bed to capture any weird episodes during the night. It took a couple nights to capture anything besides me scratching my face and arms and smacking my lips every 10 minutes all night long. What I captured was nothing like anything I have experienced before (that I know of). I wasn't clenching my fists but rather extending my left arm out and waving it side to side, with my hand in a panther punch position and a grimacing look on my face. They aren't clustered and only last a few seconds.

Here are two videos of the "episodes", I have a couple more, but basically the same thing. I was hoping to get some input or opinions on what this might be, as it does seem atypical of seizures and doesn't really look like what I had been experiencing before. https://youtu.be/yOg8KLZ_czohttps://youtu.be/hYjUW7v65d0
