Results from a new survey conducted by Kantar Health on behalf of SK Life Science, Inc., reveal that for people with epilepsy, transportation can be a significant challenge. Some people may even be restricted from driving, depending on the severity of their seizures and where they live.
We are excited to announce the relaunch of the partnership with SK Life Science, Inc., for a first-of-its-kind Uber ride-share campaign. Through this initiative, people with epilepsy or seizures can receive a $50 Uber ride-share voucher to help with urgent transportation needs to medical appointments when their regular transportation method is unavailable or cancel when needed. You must meet the following requirements to get a voucher, you must be a person with epilepsy or seizures and pre-register for the program. 

Get Your Ride Share Voucher

Complete a Ride Share program application to apply for a voucher. The Epilepsy & Seizures Helpline monitors submissions, and an information specialist will respond within one business day. Once your application is approved, you can call the Epilepsy & Seizures Helpline at 1-800.332.1000 (en Español 1-866-748-8008) during business hours to obtain a voucher.

You can also speak with an Information Specialist by calling 1-800-332-1000 Monday through Friday, 9:00 AM ET to 5:00 PM ET.

Individuals are only eligible for one (1) voucher every three (3) months. Users must provide 1) a verifiable letter from their physician indicating they are a current patient receiving in-person care for epilepsy or a seizure disorder. This letter must be on your physician's official letterhead, be less than 30 days old, and indicate the date of your next in-person visit; 2) a valid phone number; 3) a valid email address to receive a voucher code; and 4) have an active UBER account, or the ability to obtain an Uber account with an alternate method of payment to use the voucher. Vouchers are available for distribution and use in the United States only.

*Terms and Conditions: Uber voucher codes are limited to one code per person and cannot be shared or duplicated. User must have (or sign up for) an Uber account in order to apply the voucher. Vouchers are specifically designated for people with epilepsy and their caregivers. If you or a loved one do not experience epilepsy, please refrain from completing this questionnaire. Note, this voucher will expire on December 31, 2023. The voucher can be applied to one or more Uber rides, pending each individual ride amount. For example, if user’s first ride totals less than $50, the user can apply the remaining amount towards an additional ride. If a ride totals more than $50, the $50 will be applied and the user will be asked to complete payment for the remaining $ amount.
