Survey of Parents and Caregivers of Teens and Young Adults

The Child Neurology Foundation is seeking input from parents and caregivers of people from age 12 to 30 who have

• A complex health condition
• And are not expected to live fully independently as an adult

This survey is being led by the Child Neurology Foundation’s Caregiver Perspectives on Transitions Project. They would like to learn your experiences as you and your family move through different phases or changes in medical care as well as community services.
