Shave your entire head or just the incision for surgery?

Can't help you with the hair. As a male, I said, "yeah, whatever". :D They just shaved along the incision, maybe 1/8" wide and the rest of my hair covered it up. After the surgery I decided it was time for a change and buzzed it all off! You can see the incision if I point it out.
I have detailed my "non-hair-related" experiences here:

Wow, what an incredible story!!! I'm glad that it all worked out for you. It gave me a little further insight as to what I may be going through and what chances I may be taking. I think that is something that everyone needs to read who may be considering surgery. Thank you for sending that to me.
As for the hair thing, you are so lucky to be a man!
:)
