Keeping info on your condition with you

I have a notebook in my backpack that has the basic information about my epilepsy. So it has from the hospital the visits and information. Medications I take, I keep a log of my Siezures, an overview of my other medical conditions, and my boss wants a note that I can go back to work after a siezure, I keep a copy of them in there.

I also keep a small notebook on me and write down how I feel and what happening during the siezures.

I have had to go to other hospitals and first responders, they say its ahndy for them, when they ask what I take, or what I have its all right there.

Anyone else do something like that?
