This training program explores the dynamics of care coordination partnerships and how collaborative approaches can assist in health care transition planning.

At the conclusion of this training, you will develop the knowledge and skills necessary for forming effective, continuous partnerships that support and empower children, youth and families in the patient- and family-centered medical home.
