3 1/2 y/o put on Keppra - psychotic behavior?


I have a son that was diagnosed in 12/07 with Epilepsy. He’s currently taking Depakote with Zonegran. Today at our follow-up appointment we were told to drop Zonegran and replace it with Keppra (Zonegran not working). The doctor said that 2 out of 10 children experience psychotic behavior, are agitated and/or become violent...obviously this makes me nervous. Especially that my son is currently been evaluated by a child psychologist/preschool specialist on ADHD. Can any other parent relate to our situation? Someone who has a young child on this medication and might have some advice on what else I should look out for. It is really frightening putting such a young child who can't communicate what they are experiencing on such medication. Thank you!

My 14 yrs old daughter started with Petit Mal E when she was 4 yrs old.  Tried Zarontin' didn't work so went to Depakote. Did work and was on it till last year when she began experiencing another type of seizure, Myoclonic and is now off Depakote and on Zonegram.  Is working fairly well.  One note of concern about Keppra.  Our Neuro had originally recommended Keppra before the Zonegram.  After reading about the side effects I chose not to have her try it.  She was already having some serious psych. issues (very deep derpessions and wanting to hurt herself and not wanting to live).  She was  diag. at school with ADHD back in lst grade and was on concerta for years.  I agree with the post that mentions that the schools are not familiar with E and the sometimes learning issues that can  accompany it.  It is not always ADHD.  Schools and some Drs. tend to label everything ADHD these days.   Learning Disabilities are different from attention issues.  Make sure he is tested  for both. Also, how much is really attention/ hyper verses a 3 1/2 yr. old boy.  Some of that activity is very normal and the school and teachers today don't want to handle a child who may be a little bit more of a handful that the norm, but that doesn't mean they have ADHD.  I have seen kids on these meds. that are like zombies compared to their regular personalities.   The ADHD meds did help her attention, but after much trial and error and in hindsite, it was this med that was causing her depression.  WE took her off it at 12 and she is not depressed to that extreme anymore.  She has the normal teenage type stuff.  Also, she complained of chest pains all the time while on the ADHD meds.  Be careful if the Dr. and school recommends meds. for ADHD.  I find that some of the kids problems are because of overmedication.  She struggles much more in school now but at least she doesn't want to die.  I was concerned to try the Keppra b/c of her emotional issues.  Also, she was older than your son.  I see from some of the other posts that it just depends on each person as to the reaction they may have.  Trying and choosing meds is a tough decision.  Just do all the research you can on it and also, be aware of other meds he is on or are being introduced with it and keep a very good eye on his personality.  Also, if you feel he does have some learning issures, read up on your state rights and start now getting him the extra help he will need in school.  Each school area has lots of programs and help they can give you for free.  My daughter has been getting extra help since first grade.

My daughter almost 4 was on Keppra also. It definaly has moody side effects. I would keep it on the lowest possible dose if there is not another drug to choose from. You can always try it to see the effects on your son. Not every chid will react the same way. We were able to take her off that med because her seizure activity got alot better.

My son is nine years old and has been on Keppra for a year for benign rolandic epilepsy. He had numerous emotional swings with the Keppra. From extreme anger to weeping in his bed. We did research and talked to many people on this site and have started him on a vitamin B complex along with a magnesium oxide supplement. In just two weeks time we have seen a big difference. Your child being as young as he is make sure to ask your pediatrician about this and make sure there is no interference with the seizure meds. My son also developed acid reflux most likely caused by the keppra so we give him 1 oz of aloe vera juice before we eat supper. Seems to do the trick but he still has to limit the amount of ice cream, chocolate and certian canned spaghetti.

Your situation is a very difficult one.  First, I have to say that the AED's will effect people differently. I do not mean to say that this will happen to you because your child may respond wonderfully to keppra.

Two of my children have Asperger's disorder and were taking keppra for seizure control.   My son was exhibiting impulsive behavior which I thought was either a consequense of the seizure disorder or Asperger's disorder.  After 3 years of use we took him off of it when he was 8 years old and saw improvement in his impulsivity. 

 The 5 year old was also taking keppra but the behaviors were much worse.  Impulsivity, agression, running away from school and violent outbursts.  After 1 year on keppra we took her off and her symptoms also improved.

My concern after talking with many parents is that keppra will worsen the symptoms in some children who have developmental disorders and other disablities.

 However, I take keppra with carbatrol and it has improved my quality of life and seizure control.  Not to say that I do not have psychiatric symptoms from time to time, because this is common in partial epilepsy.

Your situation is a unique one and I would approach it with caution.  Since your child is only 3.5 yrs old,  it may be difficult to differentiate between behaviors due to the ADHD, epilepsy or AED.   As children get older, more demands are put on them in school, the enviroment changes and socialization becomes more difficult and they will react to these changes in different ways, some of which can be maladaptive.  Are the reactions due to the drug?  In most cases it is impossible to tell.

Would it be possible to have the testing done before your child is put on keppra?  Also, given the fact that your child has epilepsy,  it is prudent to have a neuropsychologist do an evaluation.  Most school psychologists have limited experience dealing with epilepsy and are not familiar with the psychiatric and learning problems associated with it. 

Since I am not familiar with ADHD, I am not sure of what you should look out for when your child is put on keppra. Obviously, any behaviors that are dangerous or out of character should be reported to the neuro immeditely.  I would also watch for worsening of the ADHD symptoms.

Happy Daze                      Hi my name is ducky and I have a young son who also has asperger sysdrome.  I take keppra for szs and it works good for me.  He likes school but sometimes it is difficult for him.  He is not on any meds but has problems sometimes in school.  I hope that the doctors find the right meds or your child.  Have a great day, god bless, ducky.

My daughter, Missy, is 3.5 yrs old and has been on Keppra for about 1.5 yrs with no signs of psychotic behavior.  I would be patient with your son. Let the preschool psychologist evaluate him for ADHD and find out the results.  In the meantime, don't let the doctors warning about psychotic behavior worry you.  If, after your son has been on keppra for more than a couple of weeks, you begin to notice him being more aggressive than you have seen previously, then I would bring up the behavior concern to your doctor as soon as possible.  I would also let the preschool psychologist know about beginning him on keppra, so that they are aware of the situation. 

Also, I hope that you are weaning your son off of the zonegran.  I say this because if your son has been on zonegran (or any med) for more than a week or two, it is wise to slowly transition your child off of the med.  If you take your son off of the med cold turkey or too quickly, it could actually cause withdrawal symptoms. I understand how hard it can be getting the meds to work just right.  Missy started out on topamax at 2.5 months old.  She has since been on topamax with keppra, weaned off the topamax, and had klonopin and zonegran added to her keppra to help control her seizures.  The keppra has helped control her partial seizures, however could not control her myoclonic (startle) seizures on its own.  The klonopin and zonegran have helped bring down the number of myoclonic seizures she has each day.  We are still working on getting Missy seizure-free and may have to start looking at other options (such as VNS).  However, we are staying hopeful that her seizures will come under control.

I hope that all goes well for your son and your family.  Keep logs of his seizures and any behavior changes.  And above all else, remember that he is your son and to follow your gut.  Good luck and God bless.

My son took Keppra and he has ADHD, as well as PDD-NOS, OCD & ODD. It was a couple of years ago, and we never saw any of the famous "Keppra Rage" I've heard so much about. He took it for a year or so, and we weaned him from it because it had no effect on his seizures. Just goes to show what I've learned from other parents - everyone reacts differently to drugs.

When our neuro prescribed Keppra, she warned us of the possible behavioral side-effects. We were extremely concerned and watchful of that, because our son has had problems with psychotic behavior in the past, which was unrelated to his seizure meds.

We've tried most of the AEDs available in the US, and the worst one (side-effect wise) for him was Phenobarbital, which was the very first one we tried, when he was 10 mos. old. He went from a happy, well-adjusted baby on a regular sleep schedule to a hyperactive, agitated, unpredictable little you-know-what. And it didn't control his seizures at all. We saw SUCH a difference once we got that stuff out of his system!

A word to the wise: As tempted as you may be to stop giving a drug, don't do it without a titration schedule. To stop cold turkey can be more dangerous than you think; at the very least it can cause more seizures. And keeping track of all seizure activity, side effects, & behaviors - even if you're not sure of which it is - is always worth the effort. I use spiral notebooks & calendars, (have done it for 17 yrs.) and it's proven to be an invaluable resource for us. And trust your own instincts - nobody knows your child better than you do. Not a lot of drs. will tell you that, but it's true.

Hi,

 My son is 14 years old and was on Keppra for 7 months. He went from being the sweetest kindest most gentle kid, always the teachers favorite in the class and a straight A student - to becoming so aggressive he hit me, fought physically with his older brothers daily, constantly aggitated and ANGRY, locked himself in his bedroom every second he was at  home and became totally anti social. He became so depressed that his grades dropped to all F's within 2 months of taking it. He hated his friends, school, his brothers and me. He became so bad he turned suicidal and I had to go to school and pick him up one day and went straight to the nuerologists office who urgently weened him off the Keppra and put him on Carbatrol. Within two weeks I had my son back and his grades are quickly picking up, he is laughing and socializing again and is totally lovable again. I cannot believe the difference - thank god. The carbatrol has side effects, physical not mental. But the side effects are no worse than Keppra - Physical side effects that is.

I would not recommend this for anyone.

I'm new here but my 3 yo son has been on Keppra since December of 07. This is a hard age to be on something that has such side effects as mood swings since it's also in a toddlers nature to go through various phases regardless. Half the time we wonder whether it's the Keppra, or it's a normal toddler tantrum were experiencing.

The few things that we know were NOT normal in any way were our son's night terrors and when on a higher dosage, he was hallucinating. We simply lowered the dosage and these went away.

Regardless, we're in the process of switching meds due to a breakthrough of 8 seizures within a 17 hour period. The doc's second choice for least amount of side effects is Depakote. I'm really worried about the sleepiness, but we'll see.

 Best of luck! Playing the guessing game with meds is NO fun!
