Lamotrigine and confusion

Hi, I am new to this forum.  My husband is the one with seizures.  He has been taking lamotrigine for a few years now and it controls his seizures well.  However, he complains (and I notice it too) about foggy brain.  It doesn't seem to be his memory but unclear thinking.  For example, if he has a decision to make on something, he will go back and forth on it for days sometimes.  He has always been so sharp and this can be quite disturbing to both of us.  I find that sometimes I have to explain things to him several times before he "gets" it.  The only other side effect he has from the lamotrigine is a tremor in his hands, that is quite annoying to him at times.  His dosage is 200 mg. 2x day.   Does anyone else have side effects like this?   Thanks so much!
