Relationship problems due to epilepsy

Hello, I was diagnosed with epilepsy at age 16, I am now 39. I seriously struggle with follow through and responsibility which I have struggled with all my life. I have also struggled with mood swings due to MEDS. My ex wife left me because they were out of control. For the last year and a half I have been dating an amazing girl. When we first started dating my license was suspended due to siezures. She stepped up and drove me around for six months until I could drive again. She recently told me that she doesn't think we could move forward with a family and a home because of my lack of follow through and responsibility. She also just started her masters and informed that she can no longer handle the responsibility. I get stress induced siezures and fear I will collapse. Has anyone had relationship issues due to epilepsy or does anyone have some inspirational advice?
