Video EEG monitoring results

Tue, 01/30/2007 - 12:16
My 15 year old son was admitted for 6 days in December for video EEG monitoring due to breakthrough seizures (Med: Trileptal) that had changed from his typical complex partials. WE have just rec'd the report from the epileptologist and there is some verbage/findings that stand out to me and I would like to share and get some thoughts about. During the first 2 days of monitoring, my son's medication was slowy tapered down and then stopped. Although he had no seizures the first three days, they did record 'left parietal and left anterior temporal discharges' with regularity, approximately 2-3 'discharges' per 2-5 minutes. Interestingly enough, as we began to taper down and then discontinue his medication, there was a paradoxical REDUCTION in those discharges (1-2 discharges per 2-3 hours). The only comment they have is that this is of 'uncertain clinical significance'. He did ultimately have tonic clonic seizures once the medication was out of his system completely, however, it's the little 'discharges' that are questionable for me. Obviously it concerns me that he is having 'discharges' while on medication, activity that decreases when the dosage goes down. Although these are not full blown seizures, this activity still has a big effect on his daily life, learning and energy levels. Is it possible his Trileptal is controlling the larger seizures, but he needs something else for the little 'discharges'? Am I reading too much into this? Sorry for the length, any insight or opinion would be appreciated! PS - The outcome was to increase his Trileptal to 2,700 mgs/day (over the adult recommended therapeutic dose) and he has been well-controlled so far(one day of seizures in 2 months!) Regards, CK
