Migraines/Partial Seizure

My 20 year old son has constant 24/7 brain fog and confusion. Anxiety of course makes things worse. Was diagnosed with atypical migraines. Amitriptyline worked for 2 years. Then quit working and he has 24/7 brain fog, like that weird sensation when drunk. He is in college and can't function. Went to ER yesterday and they are now suspecting mini seizures - frontal lobe partial. So confusing. He has been on Topiramate 50mg 1x/day for a month and is miserable. Going to a neurologist in Denver now as this is where he is in college. So hard to navigate a real diagnosis in a new city. On the verge of tears as he is feeling hopeless. If they are seizures - they are not visible - other than the body twitching at night. ER said they can be mini seizures and he would never know. Does any of this sound familiar symptom-wise with brain fog/confusion?
